OHIP coverage - for residents of Ontario

The application forms are available in both English and French from the Government of Ontario Forms Repository:

• Prior Approval Application for Full Payment of Insured Out-of-Country Health Services (Form 014-1442-84, in English)
• Demande d'approbation préalable du remboursement des services de santé hors pays assurés (Form 014-2204-84, in French)

Access the appropriate form online, fill in as much preliminary information as you can, and then print it. Have your physician complete the remainder of the form and then fax it with as much supporting documentation as possible, to the OHIP offices in Thunder Bay, Ontario at 807-475-1427. If your situation is urgent or extremely urgent, this should be noted in your application.
